Every eligible new staff member must receive a Standard Choice form. You can use Media Super’s form or one available from the ATO if Media Super is not your default fund.
Not registered with Media Super
If you have an employee who has nominated Media Super as their choice of fund and you're not already registered with us, please follow the instructions on our Join Media Super page.
Already registered with Media Super
If the new staff member is already a member of Media Super, we require their:
- full name, and
- Media Super membership number.
If they are not already a member of Media Super, we require their:
- full name
- full address
- date of birth
- tax file number, and
- employment start date.
Media Super can only legally accept contributions for members if there is sufficient information to create an account for them and to accurately allocate their contributions to that account. Although it is preferable for new members to complete a Member Application form, we can receive contributions for an employee as long as you provide the above information.
Once you have provided these details, they will be automatically saved within your online account.
