Use this form if wish to convert your Transition to Retirement (TTR) pension to the retirement phase. You will need to meet one of the conditions of release listed on the form.
Use this form to make your initial investment choice or change your investment choice.
Use this form if you are an existing Media Super Pension account holder and:
- you wish to invest in LifetimePlus, OR
- you are already invested in LifetimePlus and would like to request a partial or full withdrawal from LifetimePlus.
You should read the LifetimePlus Product Disclosure Statement before investing in or withdrawing from LifetimePlus.
Note: This option is now available to members with a Transition to Retirement pension account.
Use this form to nominate who you would like to receive your death benefit if you die while a member of Media Super. Your nominee(s) must be a Dependant* and/or your legal personal representative.
Note: you will not be able to change your Reversionary Beneficiary with this form.
Use this form to start a Media Super Pension or a Transition to Retirement (TTR) Pension.

Note: You must have reached preservation age before starting a Pension or TTR. Your preservation age varies between 55 and 60 years, depending on your date of birth.
